New Patient Information
Please select the "Book Now" button below to schedule your first initial visit. We ask that you fill out the New Patient Forms prior to your first appointment. This allows our doctors time to analyze all problems and symptoms you’re having. If you prefer to complete a paper copy of your New Patient Forms please present to the office at least 10 minutes prior to your scheduled appointment time.

Insurance Coverage:
Dr. Kamara is in network with various PPO insurance carriers including BlueCross BlueShield of IL and Medicare. Please contact our office if you have any questions regarding chiropractic coverage with your insurance. We do accept all insurances, even if we are not an in-network provider.
